My daughter is a college student and took an uber for $40 to this appointment. When she got there they said they wouldn't see her because she didn't fill out paperwork... they never sent us paperwork

Appointment was scheduled for 11:15 am, left after 11:58 am without being seen. Extremely unprofessional staff and lack of urgency amongst all workers. Do not waste your time.
