the entire restaurant was filthy, my mother witnessed a female employee come out of the bathroom stall walk out of the bathroom without washing her hands and proceed to touch people's food and orders.

Beef 5 layer burrito is a scam. There is no beef in the burrito. Need to bring back the Taco Bueno chains. They had real beef and the picante sauce was freshly made in the store, not in a package.
